When is a Life Care Plan Needed?

A Life Care Plan is  indicated for individuals who are living with a chronic illness or have sustained a catastrophic injury, and may include any of the following:

Case Management

Nurse case management services are often required for individuals with significant and complex medical issues.  Ms. Murphy has provided case management services for both pediatric and adult individuals, with complex medical and social needs, to optimize the quality of life and well-being of the client and family.
